ConnectCare NY helps New York City Medicaid members find out if they can get free healthy meals or groceries, and takes care of the paperwork with their health plan.
What we do
We talk with you, check with your Medicaid health plan whether you qualify, and send the referral. Approved food providers then deliver the meals or grocery boxes to your home. The service is free for people who qualify.
Who we help
Adults, older adults, families and pregnant members across Brooklyn, Queens, the Bronx, Manhattan and Staten Island. Our team speaks English, Spanish and Chinese.
How we handle your information
Your information is encrypted, only staff who need it can see it, every view is logged, and it is never sold or used for ads. You can ask for a copy or ask us to delete it at any time.
